The Patient-Centered Outcomes Research Institute (PCORI) fee is an annual fee imposed on health insurers and self-funded plan sponsors. For plan years ending on or after October 1, 2025, the fee is $3.22 per covered life. Self-funded employers report and pay the fee on IRS Form 720.
