Choose the strategy before the vendor

Specialty pharmacy vendors use fundamentally different models. Understanding what each model does — and what risks it carries — is more important than comparing vendor names.

Alternative Access / Alternative Funding

Helps members access manufacturer patient-assistance programs or other non-plan channels for high-cost drugs. The plan may exclude the drug from coverage; the vendor helps the member get it another way.

Requires plan-document changes, fiduciary review, and stop-loss carrier approval. Manufacturer programs can change.

Copay Maximizer / Copay Optimization

Captures manufacturer copay-assistance dollars on behalf of the plan. Reduces member out-of-pocket on targeted specialty drugs while shifting more cost to the manufacturer.

Subject to ongoing litigation and regulatory scrutiny. HSA/HDHP and accumulator treatment require careful review.

Specialty PBM / Clinical Carve-Out

Carves specialty drugs out of the traditional PBM and manages them through a dedicated specialty PBM with clinical protocols, formulary management, and biosimilar-first policies.

Requires PBM contract renegotiation. Limited-distribution drugs and network access must be verified.

Procurement / International Sourcing
