Direct Primary Care (DPC) for Self-Funded Employers

Direct Primary Care is a membership-based model where employers pay a flat monthly fee for unlimited primary care access — no claims, no copays, no insurance billing. It's one of the most effective ways to reduce ER visits, unnecessary specialist referrals, and total healthcare costs in a self-funded plan.

How employer-sponsored DPC works

The employer pays a monthly membership fee (typically $50–$100 per employee per month) to a DPC practice. Employees get unlimited access to their primary care physician — same-day or next-day appointments, extended visit times, direct messaging, and often basic labs and procedures included in the membership.

Because DPC physicians aren't billing insurance, they can spend more time with patients, manage chronic conditions proactively, and handle issues that would otherwise result in ER visits or specialist referrals. The result is better care and lower total cost.

The cost impact

30–50%
ER visit reduction
DPC members have significantly fewer ER visits because they can reach their doctor directly.
20–40%
Specialist referral reduction
DPC physicians handle more in-house and refer only when truly necessary.
15–30%
Hospitalization reduction
Proactive chronic disease management reduces preventable hospitalizations.
$1,000–$3,000 PEPM
Total cost savings
Estimated annual savings per enrolled member from reduced downstream utilization.

DPC + high-deductible plan design

DPC pairs naturally with a high-deductible health plan (HDHP). The DPC membership covers primary care outside the insurance plan, so employees rarely hit their deductible for routine care. The HDHP provides catastrophic coverage. Together, they deliver comprehensive coverage at significantly lower total cost than a traditional low-deductible plan.

What to look for in a DPC partner

Panel size — DPC physicians typically limit to 400–600 patients (vs. 2,000+ in traditional practices)
Geographic coverage — enough locations to serve your workforce
Technology — direct messaging, telehealth, easy appointment booking
Scope of services — what's included in the membership vs. billed separately
Employer reporting — utilization data to demonstrate ROI
Integration with your TPA and benefits ecosystem
