A provider network is the set of healthcare providers — physicians, hospitals, labs, imaging centers — that have contracted with a health plan to provide services at negotiated rates. In-network providers have agreed to accept the plan's payment rates; out-of-network providers have not. Network adequacy (whether the network includes sufficient providers in the right specialties and geographies) is a key plan design consideration.
