A level-funded plan is a hybrid between fully insured and self-funded. The employer pays a fixed monthly amount covering expected claims, stop-loss insurance, and administrative fees. If actual claims come in below projections, the employer receives a refund of the surplus. Level-funded plans are popular with small and mid-sized employers (25–200 employees) as a lower-risk entry point to self-funding.
