Coalition Direct Contracting Mechanics
Coalition direct contracting aggregates the purchasing power of multiple employers to negotiate directly with providers. A coalition of 10 employers each with 500 employees has the purchasing power of a 5,000-employee employer — enough to negotiate meaningful direct contracts with major health systems. The coalition negotiates the contract; individual members direct their employees to the contracted providers.
Member Commitment
Coalition direct contracting requires member commitment to direct employees to contracted providers. Without member steerage, the coalition cannot deliver the volume commitments that justify provider discounts. Members must be willing to implement incentives (reduced cost-sharing at contracted providers) and communication programs that drive utilization to contracted providers.
High-Value Contracting Targets
IQ Insight
Coalition direct contracting is most effective for high-volume, high-cost services: hospital inpatient, ambulatory surgery, imaging, and specialty care. A coalition that negotiates a direct contract with the dominant health system in a market can achieve 30–50% savings on hospital services for all members. This is the highest-value application of coalition purchasing power.
